Services
Who We Serve:
Eligible adults who are at risk of institutionalization due to economic, physical or psychosocial reasons
What We Provide:
Daily living assistance (light cleaning, cooking, personal hygiene care, referrals to appropriate resources, etc.) are provided in the individual’s home one time per week, at 2 hours per home visit
All services are voluntary.
Eligibility Requirements
- Application
- Birth Certificate/ Affidavit of Birth
- Social Security Card
- Certificate of Indian Blood
- Driver License or State Identification Card
- Recent Bank Statement
- Verification of Residency (can be obtained at individual’s Navajo Nation chapter within which they reside)
