Who We serve
Provides daily living assistance to eligible adults who are at risk of institutionalization due to economic, physical or psychosocial reasons.
What We Provide
Daily living assistance (light cleaning, cooking, personal hygiene care, referrals to appropriate resources, etc.) are provided in the individual’s home one time per week, at 2 hours per home visit.
How Services Occur
The services are voluntary.
Eligibility Requirements
- Application
- Birth Certificate/ Affidavit of Birth
- Social Security Card
- Certificate of Indian Blood
- Driver License or State Identification Card
- Recent Bank Statement
- Verification of Residency (Can be obtained at individual’s Navajo Nation chapter within which they reside.)
